New Patient Registration

New applicants to join the Practice should make a routine appointment through the receptionist. You can complete our New Patient Registration form via our website and bring this with you when you attend for your appointment

On registering with the Practice you will be invited to have a New Patient Appointment to record routine clinical information about yourself and for any necessary examinations to be carried out. This will normally be arranged within 6 months of you registering with the Practice but can be done anytime subsequent to that at your request. The examination will normally take place at the surgery but in exceptional circumstances if your doctor considers it necessary this can be carried out in your home.

Please complete the new patient registration form and bring it with you to the surgery.

Proof of your Identity

Proof of your identity must be brought to your GP appointment.   Examples of identification are detailed on your registration form.   Failure to do so will result in you being turned away.   You will not be seen by a Doctor and will need to approach another GP Practice in the area.

Medication

Please note you are still registered with your previous GP until you have attend the Practice for your new patient appointment and are fully registered on the system.

Please ensure you contact your previous GP to obtain enough medication to last until you have registered with the Practice.

Patient Preference

When you are accepted on to the Practice list you will not be allocated to a specific doctor. However, you have the right to express a preference to receive services from a particular partner or partners and to have this recorded in writing at the Practice. We will endeavour to comply with such a request but this may not always be possible.

This Practice operates a zero tolerance policy towards violence or abuse to staff.
