Non-urgent appointments can be made in advance by telephoning 01333 432588 or in person at Leven Health Centre.
Routine appointments can usually be made up to one month in advance.
Due to demand we are operating a triage system for urgent or same-day requests for appointments. These requests should be made by telephone. It is helpful if you give the receptionist as much information as possible. A clinician will telephone you back and discuss your problem with you. They will either give you appropriate advice, an appointment with a Nurse, or an appointment with a Doctor depending on the nature of your problem.
Our triage telephone calls will appear as a withheld number. This is currently outwith our control. Please ensure your telephone can accept withheld numbers and let the receptionist know if you are unable to change it.
Patients should be aware that each person requiring to be seen requires a separate appointment.
Please also show consideration to other users of the service by informing the reception staff if an appointment cannot be kept so that it can be offered to someone else.
Patients over the age of 16 years and under the age of 75 years who have not consulted a doctor or attended a nurse led clinic within the past three years are entitled to request a consultation for a routine check-up.
Patients over the age of 75 years who have not consulted a doctor or attended a nurse led clinic within one year are similarly entitled to request a consultation.
